January 6: Final Document Turn-In Day

Dear Parents,

Thank you for registering for the spring season with the Patriot League! The final document Turn-In Day is Saturday, January 6, 2024, from 9:00 a.m.-12:00 p.m. at Shira Field (7001 Murray Park Dr., Field #7).  Players do not need to attend Document Turn-In Day. Players who do not have all documents turned in to complete their registration by the last Document Turn-In Day will not be assigned an assessment day/time and may risk being moved to the wait list.

PLEASE DO NOT UPLOAD YOUR DOCUMENTS TO THE WEBSITE OR EMAIL THEM TO US. To complete your player's registration it is imperative we receive hard copies of all of the documents listed below.  Also, there are far too many players and scheduling situations/conflicts to assign assessment times via email so once your player's documents have been turned in and the birth certificate has been verified an assessment day/time will be scheduled. Turning in your completed documents on Document Turn-In Day helps the league tremendously in preparing for the season on multiple levels. 

If your player HAS NOT been an All-Star in the past, and even if you completed any of these forms when registering for Fall Ball, please be sure to bring the following completed documents (and Proofs of Residency or School Enrollment Form, if applicable) as they are required each season:
1. Registration Form
 
2. Medical Release

3. Photo/Media Release
 
4. Concussion Form
  (State Mandated Form - needs signed by both the parent and player)
5. Sudden Cardiac Arrest Form
  (State Mandated Form - needs signed by both the parent and player)
6. 
 D33 Code of Conduct
7. Patriot League Volunteer Agreement
8. Original birth certificate. We do not keep the birth certificate, but need to see for validation purposes.

9. Three proofs of residency from the three different groups (one from Group I, one from Group 2 and one from Group 3 as detailed in the Residency and School Eligibility Requirements
 ) if using the home address to establish eligibility OR  School Enrollment Form  (if the player is using the school address to establish residence eligibility)

IF YOUR PLAYER HAS BEEN AN ALL-STAR (and if we are in possession of his/her All-Star packet), and nothing has changed regarding the status of the home address or school of attendance then only documents 1-7 are neededIf the player now attends a different school than when originally certified or has moved since last season, then please advise so a II(d) waiver can be processed by the league.
